Anthony Grainger was sitting in the driver’s seat of a stolen car when he was shot in the chest by armed police.
Officers believed he had been planning an armed robbery and had a gun but they were wrong on both counts.
The chief constable of Greater Manchester police said he was “sincerely sorry” for what an inquiry described as a “catastrophic series of errors”.
